Job Description:

As an Outpatient Diabetes Educator, you will work in partnership with Health System to educate patients with diabetes in outpatient settings. You will also collaborate with other clinicians to identify patients who would benefit from diabetes education and management. This role will be based at Ascension Saint Thomas Rutherford Hospital in Murfreesboro, TN.

Responsibilities:

* Assess patients' understanding of diabetes and chronic complications
* Provide group and individual education on diabetes management
* Work with physicians and other providers to identify diabetes management needs
* Identify and support continuing education for outpatient office clinicians
* Act as a resource to program staff, nursing, and ancillary department staff

Requirements:

* Registered Dietitian
* Certified Diabetes Care and Education Specialist (CDCES) within two years of employment
* Appropriate professional licensure/registration
* Bachelor's degree
