This role involves delivering exceptional nursing care across various community settings. The successful candidate will adhere to the philosophy, policies, and standards of practice set forth by Partners Healthcare System.

In collaboration with the RN case manager, you will conduct skilled nursing visits, ensuring thorough and timely electronic documentation while maintaining effective interdisciplinary communication in accordance with established agency protocols.

Qualifications and Experience

  • Valid LPN license in Massachusetts
  • At least 1 year of nursing experience in acute or sub-acute care environments is required
  • Experience in home care is preferred
  • Proficient written, verbal, and electronic communication skills
  • Familiarity with various computer software is essential; knowledge of handheld patient care devices is a plus
  • Ability to communicate effectively across multiple formats
  • Demonstrated knowledge and skills necessary to provide care to diverse populations, including geriatric, pediatric, and adult patients
  • Strong interpersonal skills with the ability to engage with a varied demographic
  • Capability to function as part of an interdisciplinary team
  • Excellent organizational skills, with the ability to work independently and adapt to changing circumstances
  • Local travel is required
