- Coordinate discharge planning to ensure a smooth transition for patients.
- Provide psychosocial interventions to address complex health needs.
Requirements:
- Master's in Social Work required.
- LMSW required; LCSW/LCSW-C preferred.
- 3-5 years of experience in social work.
- MD Social Work License per level of education.
