The Department of State Hospitals is seeking a skilled Nurse Practitioner to join our Patient Management Unit. As a key member of our team, you will play a critical role in providing exceptional care to patients and contributing to the success of our preadmission pathway.

Key Responsibilities

  • Participate in Care Coordination efforts to ensure seamless patient transitions
  • Provide evaluation and consultation to aid in the appropriate placement of patients
  • Collaborate closely with other team members to provide exceptional service to assigned counties
  • Work under the direction of the Nurse Consultant, Program Review of the Patient Management Unit

Working Conditions

  • This position is physically located in Sacramento and is currently offered with the potential for a hybrid work environment
  • Travel throughout California for site visits, including overnight travel, to DSH partnered treatment facilities will be required with prior notice

Requirements

  • Applicants must submit the required documents in order to be considered for this position
  • Failure to complete the STD 678 in its entirety may result in disqualification
