Role: Physical Therapist
Summary:
The Physical Therapist is responsible for delivering services aimed at identifying, preventing, and rehabilitating physical dysfunction or pain. This role involves evaluating patients, applying therapeutic techniques, and collaborating with patients and their families to develop personalized care plans. Effective communication with the overseeing physician and other healthcare professionals is essential to ensure a cohesive approach to patient care.
Key Responsibilities:
1. Conduct comprehensive physical therapy assessments, including evaluations of functional limitations and mobility status.
2. Identify additional home health service needs for patients and facilitate referrals as necessary.
3. Implement treatment plans as prescribed by the physician, ensuring clear communication with all involved healthcare providers.
4. Administer appropriate therapeutic interventions to alleviate pain, enhance function, and promote independence using various modalities.
5. Maintain detailed records of evaluations, treatments, and patient responses to therapy, contributing to discharge planning.
6. Engage in case management discussions with interdisciplinary team members to ensure coordinated care.
7. Provide education and counseling to patients and families regarding treatment plans and document the delivery of this information.
8. Ensure the availability of necessary equipment to support patient rehabilitation and independence.
9. Participate in quality assurance initiatives and ongoing training programs.
10. Supervise the work of Physical Therapy Assistants and home health aides as appropriate.
11. Adhere to all compliance standards relevant to the role.
12. Perform additional duties as assigned.
Qualifications:
Required:
- Bachelor's degree in Physical Therapy
- Minimum of one year of relevant experience
- Current Physical Therapist license
Preferred:
- Experience within a recognized home health agency.
Working Conditions:
Essential:
- Requires significant manual dexterity and coordination.
- Involves substantial physical effort, potentially exceeding 125 lbs. of force.
- May include exposure to various environmental factors.
