Key Responsibilities- Utilize clinical knowledge and skills to care for individuals with physical disabilities and chronic conditions.
- Engage in patient and family education, employing appropriate resources to create personalized teaching and discharge plans.
- Deliver hands-on nursing care through the nursing process to achieve optimal patient outcomes.
- Support and reinforce care provided by fellow team members.
- Guide and supervise unlicensed assistive personnel, demonstrating clinical judgment aligned with Iowa's LPN scope of practice.
- Coordinate nursing care activities in collaboration with the interdisciplinary rehabilitation team to meet overall objectives.
- Exhibit effective communication skills to build rapport with patients, families, and healthcare team members, ensuring compliance with legal documentation and reimbursement requirements.
- Maintain a strong commitment to the Acute Care team.
- Contribute to the assessment of patients' physical, psychological, sociocultural, and spiritual needs, as well as their educational and discharge requirements.
- Collaborate with rehabilitation team members to enhance the interdisciplinary care plan.
- Implement the interdisciplinary care plan within the LPN/LVN practice scope, providing nursing care and education as necessary.
- Participate in evaluating nursing care and modifying plans to achieve measurable goals.
- Engage in interdisciplinary team processes during conferences and meetings, contributing to team decision-making within your practice scope.

Education:
Completion of an accredited licensed practical nurse program.
Experience:
Clinical experience in Post-Acute Care or Acute Care settings.
Licensure/Certification:
Current licensure in good standing as a Licensed Practical Nurse in Iowa.
Must maintain current Basic Life Support (BLS) certification or obtain it within three months of hire.
Valid driver's license required for work-related travel.
Knowledge, Skills, and Abilities:
Proficient in reading, writing, and speaking fluent English.
Multicultural sensitivity.
Basic computer skills, including Microsoft Office.
Customer and patient-focused approach.
Critical thinking abilities and independent judgment in decision-making.
Other:
Familiarity with standard equipment necessary for performing essential job functions.
Occasional travel to other facilities may be required, utilizing a UPH vehicle, rental, or personal vehicle.
