The RN Case Manager will be based out of our Portsmouth office and serve the Greater Seacoast community.

The RN Case Manager is responsible for managing a defined caseload of patients, providing direct patient care, and comprehensive nursing services to individuals in the community.

Open positions include Full Time, Part Time, and Per Diem.

Hours:

Monday-Friday 8:30am-5pm (Flexible days if desired)

Job Requirements of the Home Health Registered Nurse Case Manager:

  • Valid Nursing License in the State of New Hampshire
  • Nursing experience required
  • Current CPR Certification required (We offer monthly renewal classes paid for by Interim HealthCare)

A Sign on Bonus of $5,000 is offered to RN Case Managers that have two years of current nursing experience working at least 30 hours per week. After 90 days of employment, 20% of the sign on bonus will be paid out. The remaining sign on bonus will be paid after 6 months of employment.
