Primary Care Provider for Seniors and Complex Needs

We are seeking a compassionate Nurse Practitioner to join our team at Abode Care Partners, providing high-quality medical care and support to seniors and patients with complex needs in the comfort of their own homes.
This is an excellent opportunity for a licensed Nurse Practitioner (NP) looking to make a meaningful impact in the lives of our clients. You will have the chance to grow your medical skills while collaborating with other healthcare professionals to ensure comprehensive care.
Responsibilities:- Perform evidence-based medical evaluations, examinations, and treatments for chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
- Provide referrals to other healthcare practitioners when necessary.
- Ensure patient medical records are kept up to date.

Requirements- Active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- Competitive compensation: $100,000 - $120,000 per year.
