PRN Weekend Registered Nurse

About the Role:We are seeking a skilled and compassionate PRN Weekend Registered Nurse/RN Field Nurse to join our team. As a key member of our hospice care team, you will provide end-of-life care and services to patients in their homes or community settings. Your primary responsibilities will include:
- Providing intermittent skilled nursing services and directing, supervising, and instructing hospice aide staff in the provision of personal patient care.
- Assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team.
- Reporting on-call activities to clinical leadership and Branch RN Case Managers at the end of the call shift.
- Maintaining and completing appropriate care coordination notes for visit notes and other pertinent clinical information.
- Assisting, providing, or arranging for delivery of medications and supplies needed after business hours, weekends, and/or holidays.
To be successful in this role, you will need:
- Two years' experience within hospice.
- Certified Hospice and Palliative Nursing.
- Electronic Medical Records Experience.
