Responsibilities
- Assess physician medication orders for appropriateness of drug selection, dosage, potential interactions, and administration routes. Recommend therapy modifications to physicians and facility staff as necessary.
- Maintain a comprehensive patient database, including relevant medical history, clinical data, and current medications (including over-the-counter and home remedies).
- Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with relevant healthcare team members.
- Collaborate with the healthcare team to provide clinical and technical support as required.
- Document all communications with physicians, nurses, and other healthcare team members, as well as patients and their families, in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and disposal of medications and biologicals.
- Obtain and maintain all necessary documentation in compliance with federal and state regulations and company policies.
- Notify the Pharmacy Manager of any medication-related errors.
- Assist facility staff with procedures related to the safe and effective management of medications.
- Perform additional tasks as assigned.
Qualifications
- Bachelor's Degree in Pharmacy; Pharm D preferred
- Minimum one year of experience in long-term care
Licenses/Certifications:
As required by regulatory standards and business needs
Detail-oriented, goal-driven, team player with strong oral communication skills.
