We are seeking a skilled Physical Therapist to join our team at Self-Help for the Elderly. As a Physical Therapist, you will play a vital role in providing high-quality physical therapy services to our patients, promoting their independence and well-being.
Job Summary:The Physical Therapist will be responsible for assessing and evaluating patients' physical abilities, developing and implementing individualized treatment plans, and providing education and training to patients and their families. The ideal candidate will have excellent communication and interpersonal skills, be able to work independently, and have a strong commitment to providing exceptional patient care.
Key Responsibilities:- Assess and evaluate patients' physical abilities and develop individualized treatment plans
- Provide physical therapy services, including exercises, stretches, and other interventions
- Education and training of patients and their families on physical therapy techniques and strategies
- Collaborate with other healthcare professionals to ensure comprehensive patient care
- Document patient progress and maintain accurate records
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Master's degree in Physical Therapy from an accredited program
- Current licensure to practice as a Physical Therapist in the state of California
- At least two years of experience as a Physical Therapist, preferably in home health or geriatric care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong commitment to providing exceptional patient care
