Responsibilities

The Licensed Speech Therapist is responsible for delivering speech therapy services as part of the personal care provided to patients.

Key Responsibilities Include:
  • Conducting assessments and evaluations of patient needs, collaboratively establishing goals for the patient care plan.
  • Providing speech therapy services, including assessment, evaluation, procedures, teaching, and training activities as specified in the patient care plan.
  • Reporting patient progress to the Clinical Supervisor and referring Physician.
  • Documenting daily updates and changes to patient care plans, including discharge planning.
  • Participating in staff meetings, team conferences, and educational in-services as required by the agency.
  • Performing all OASIS time point assessments in accordance with Medicare criteria and submitting recertification paperwork as per policy.
  • Ensuring the application of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) while providing Age-Friendly Care.
Qualifications
  • A current, unencumbered State Professional License.
  • A valid driver's license and proof of insurance.
  • A minimum of one year of experience as a professional Speech Language Pathologist in home care, with competency in conducting comprehensive assessments and OASIS-C.
  • The ability to exercise sound professional clinical judgment.
  • Capability to assess and document patient needs, formulating individualized care plans accordingly.
  • Proficiency in clinical skills and Microsoft Office.

Compensation packages are tailored based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
