Position Overview

The Case Management Coordinator will oversee the daily operations of the Ryan White Part B Case Management Program. This role involves delivering medical case management services as part of the Kentucky HIV/AIDS Care Coordination Program (KHCCP). Responsibilities include conducting social service assessments and facilitating connections for HIV-positive individuals to essential services provided by various community programs at the federal, state, and local levels.

Key Responsibilities
  • Lead and manage the Ryan White Part B Case Management Program.

  • Connect clients with necessary resources and services.
Qualifications
  • Bachelor's degree in social work or a related discipline.
  • A minimum of 2 years of leadership experience in a professional setting.
  • Experience in a nonprofit organization or a strong desire to work in a service-oriented environment.
  • Complete alignment with the mission, vision, and values of Matthew 25 AIDS Services.
  • Experience working with diverse populations and the ability to provide status-neutral services.
