About the Role:
The Patient Care Coordinator will be responsible for maintaining a professional image and exhibiting excellent customer relations to patients, visitors, physicians, and co-workers. You will assess needs of patients as assigned based on data collected through history, observation, physical examination, and analysis of diagnostic data. Your primary objective will be to plan individualized patient care to meet the holistic age-specific needs of patients, providing appropriate nursing interventions consistent with the Plan of Care and standards of practice.
Key Responsibilities:
- Maintain a professional image and exhibit excellent customer relations to patients, visitors, physicians, and co-workers.
- Assess needs of patients as assigned based on data collected through history, observation, physical examination, and analysis of diagnostic data.
- Plan individualized patient care to meet the holistic age-specific needs of patients.
- Provide appropriate nursing interventions consistent with the Plan of Care and standards of practice.
Requirements:
- RN license
- BLS (Required for ALL nurses)
- Advanced Resuscitative Certification appropriate to their patient population if serving in a Charge Nurse or PCS position
- Specialty certification preferred
