We are seeking a skilled and compassionate Community Health Nurse to join our team at Northwell Health. As a Community Health Nurse, you will provide high-quality, patient-centered care to individuals in their homes and communities.
Key Responsibilities- Assess and evaluate the health care needs of patients and families, considering physiological, psychological, social, and environmental factors.
- Identify and implement complete, accurate, and logical care plans in collaboration with the primary care physician and other healthcare professionals.
- Participate in comprehensive care planning meetings and collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Implement nursing care plans and revise them as necessary, based on regular assessments and evaluations of patient needs and responses to care.
- Administer medications and treatments as prescribed by the primary care physician and perform skilled nursing procedures as needed.
- Teach patients and families/caregivers self-care techniques and provide instruction on medication, diet, safety, and treatment modalities.
- Delegate responsibility appropriately and supervise ancillary personnel to ensure quality care and compliance with care plans.
- Complete, maintain, and submit all required documentation in a timely, accurate, and relevant manner.
- Education: Degree from an NLN-accredited school of nursing required, BSN preferred.
- License: Current New York State license to practice as a Registered Nurse.
- Minimum of one (1) year general clinical nursing experience in a certified home health agency (CHHA), acute care, medical surgical, and/or critical care experience required.
- Minimum of one (1) year of experience working with a frail or elderly population preferred.
- Current Driver's License, required insurance, and car available for work as required.
