Job Summary

HomeWell Care Services is seeking a skilled Registered Nurse to join our team as a Nurse Care Manager. This role is responsible for coordinating, implementing, and supervising the delivery of services to our clients in alignment with our company's mission and standards of care.

Key Responsibilities

• Evaluate referred clients for appropriateness of admission and coordinate the clinical intake and business forms necessary to onboard new clients.
• Develop and implement a written care plan for each client with input and approval by the client and/or designated client representative.
• Supervise client care provided by agency caregivers.
• Regularly communicate with the client and/or designated client representative to follow-up on service delivery and ensure client satisfaction.
• Perform scheduled and non-scheduled in-home client support visits.
• Conduct reassessments according to the agency's established frequency and as needed due to a change in client condition or recent event.
• Participate in the coordination of services when more than one community service is providing care to a client.
• Serve as a resource for clients and families, facilitating client education, empowerment, and quality of life; provide recommendations and referrals as needed to third-party services.

Requirements

• Must be a licensed Registered Nurse in the State of Florida; a Bachelor's Degree in Nursing strongly desired.
• 1+ year of experience as a Registered Nurse in a home health care setting.
• Experience with Microsoft Office, sound computer skills, and strong organizational abilities.
• Excellent interpersonal relations abilities. Excellent telephone skills.
• Able to work independently, demonstrating sound judgment.
• Read, write, speak, and understand English as needed for the job. Fluency in a second language is a benefit.
• Valid driver's license and reliable vehicle with valid insurance.
• Must have an active State of Florida Level 2 Background Check or pass one.
• Be available as required for on-call duty outside of normal office hours.
