Program Overview

- The team consists of 8 physicians, 3 Nurse Practitioners, 4 Bereavement Coordinators, and 9 Social Workers, providing a collaborative and supportive work environment.
- The organization is certified as HIMSS EMRAM Level 7 for both inpatient and ambulatory facilities, ensuring a high level of electronic medical record adoption and integration.
Job Responsibilities
- Provide high-quality palliative care services to patients and families in a variety of settings, including inpatient and outpatient.
- Participate in the development and implementation of the outpatient palliative care program.
- Collaborate with the interdisciplinary team to provide comprehensive care and support to patients and families.
- Contribute to the growth and development of the palliative care program through leadership and educational opportunities.
Requirements
- Board certification or eligibility in Palliative Medicine, with fellowship training preferred.
- Significant experience in palliative medicine, oncology, hospice, or geriatrics, with a strong track record of providing high-quality patient care.
- Ability to work collaboratively as part of a multidisciplinary team and contribute to the growth and development of the palliative care program.
