Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as an On-Call Registered Nurse. As a key member of our hospice care team, you will provide high-quality patient care and support to patients and their families in their homes, assisted living facilities, and hospitals.

Key Responsibilities
  • Provide comprehensive nursing care to patients with complex medical needs, including symptom management and pain control.
  • Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams, including social workers, chaplains, and volunteers, to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making and optimal care outcomes.
  • Manage on-call responsibilities, including responding to urgent patient needs and providing support to patients and families after hours.
Requirements
  • Current Registered Nurse licensure in good standing in the state(s) of practice.
  • At least one year of experience as a Registered Nurse, with a strong background in hospice care or palliative medicine.
  • Excellent clinical and communication skills, with the ability to work effectively in a fast-paced, dynamic environment.
  • Strong time management and organizational skills, with the ability to prioritize multiple tasks and responsibilities.
  • Ability to learn and utilize electronic medical records systems.
  • Valid driver's license and reliable transportation, with a willingness to travel across the service area as needed.
