Key Responsibilities- Conduct comprehensive assessments to identify patient needs and develop individualized plans of care.
- Collaborate with patients, families, and healthcare providers to ensure seamless transitions of care and prevent hospital readmissions.
- Utilize evidence-based clinical guidelines and protocols to promote quality and efficiency in healthcare delivery.
- Develop and implement processes to monitor patients across the health continuum and optimize resource utilization.
- Provide education and support to patients and families to promote self-management and disease prevention.
- Graduate of an accredited school of nursing with a current Georgia RN license.
- Minimum 5 years of RN clinical experience, preferably in case management, hospice, dialysis, heart failure, or ambulatory care.
- Strong computer skills, including Microsoft Office and electronic health records.
- Ability to work autonomously and as part of a multidisciplinary team.
