Key Responsibilities:
">- Provide wraparound support to expectant mothers and parents of children aged birth to five through an array of services.
- Collaborate with professionals, community partners, and referral sources to deliver relationship-focused, hope-centered, and trauma-informed service delivery.
- Promote and enhance the overall health and well-being of clients through evidence-based services.
- Maintain effective communication with clients and community partners, ensuring timely referrals and linkages to resources.
Requirements:
">- Bachelor's degree in a behavioral health field or related discipline.
- At least one year of experience working directly with adults, families, or children.
- Excellent problem-solving, time management, and organizational skills.
- Able to work effectively as part of a collaborative team.
Preferred Qualifications:
">- Masters degree in a behavioral health field or related discipline.
- Behavioral Health Case Management II (BHCM II) Certification through the Oklahoma Department of Mental Health and Substance Abuse Services.
- Community Health Worker Certificate.
