At Professional Case Management, we're seeking dedicated Licensed Practical Nurses (LPNs) to join our team of home healthcare professionals. As an LPN with our organization, you'll have the opportunity to make a meaningful impact on the lives of our clients, providing personalized care and support in the comfort of their own homes.
About the Role:
- Deliver high-quality patient care with a focus on promoting health, well-being, and independence.
- Build strong relationships with clients and their families, providing emotional support and guidance.
- Work collaboratively with our interdisciplinary team to develop and implement personalized care plans.
- Stay up-to-date with industry developments and best practices through ongoing education and training.
Requirements:
- Graduate of a state-approved school of professional nursing
- Current, unrestricted license as an LPN/LVN in the state(s) of practice
- New graduate nurses are encouraged to apply, with paid mentorship opportunities available
- Current CPR certification and TB test
