We are a clinician-led care organization dedicated to creating a seamless health journey for patients across the care continuum. As a member of our Home and Community Care team, you will play a vital role in bringing home-based medical care to complex, chronic patients, helping them to have more days at home.
Key Responsibilities:- Provide comprehensive assessments and develop patient management care plans
- Collaborate with interdisciplinary teams, PCPs, and other healthcare professionals to ensure high-quality patient care
- Prescribe diagnostics and interventions to prevent unnecessary acute admissions
- Perform basic procedures of nursing care, including IV placement, blood draws, and wound care
- Consult with hospital and emergency teams to ensure seamless patient care
- Educate patients and caregivers on disease management and wellness
- Participate in ongoing meetings and quality initiatives to promote clinical excellence
- Completion of a Nurse Practitioner accredited program
- Current state RN and NP licenses
- Access to reliable transportation
