Del Sol Medical Center is seeking a highly skilled Senior Case Management Professional to join our team. This 13-week contract opportunity offers a unique chance to work in a dynamic environment, providing exceptional patient care and collaborating with a multidisciplinary team.
About the RoleWe are looking for an experienced RN with at least 2 years of Acute Case Management experience to provide case management services for both inpatient and observation patients. The ideal candidate will have strong communication skills, be able to remain calm in crisis situations, and possess the ability to build trust and deliver information clearly and concisely.
Key Responsibilities- Provide case management services for inpatient and observation patients assigned
- Collaborate with RN case managers and the multidisciplinary team to ensure comprehensive patient care
- Develop and implement effective case plans to achieve optimal patient outcomes
- Conduct thorough assessments of patients' needs and develop individualized plans to address these needs
To be successful in this role, you will need:
- Current RN licensure
- At least 2 years of Acute Case Management experience
- Strong communication and interpersonal skills
- Ability to remain calm in crisis situations and prioritize workflow effectively
