Job Description:
As a Substance Use Case Manager with Lund, you will play a critical role in our Regional Partnership Program, working collaboratively with DCF Family Services to support caregivers impacted by substance use. Your primary responsibilities will include:
  • Developing and implementing individualized case plans to address substance use issues.
  • Collaborating with multidisciplinary teams to ensure seamless care coordination.
  • Providing consultation and training to DCF staff on topics related to substance use and treatment services.
  • Liaising between the treatment system and DCF Family Services to ensure effective communication and collaboration.

Required Skills and Qualifications:
To be successful in this role, you will need:
  • A minimum of an Associate's degree in Social Work, Counseling, or a related field (Bachelor's preferred).
  • Experience providing case management services in a substance use, recovery, or mental health treatment setting (desired but not required).
  • The ability to obtain the Apprentice Addiction Professional (AAP) certification within the first year of employment (with financial support and clinical supervision provided by Lund).
  • Strong knowledge of community resources and the ability to build relationships with local partners.
