Key Responsibilities- Act as a Case Manager within the Regional Office, focusing on healthcare advocacy for incarcerated individuals.
- Maintain comprehensive and confidential patient records and reports.
- Collaborate with healthcare professionals to assess, plan, implement, and evaluate patient care strategies.
- Adjust treatment plans based on patient responses and clinical conditions.
- Oversee all facets of patient care within the facility.
- Evaluate referrals for external treatment to ensure appropriate resource utilization.
- Serve as a liaison with external healthcare providers, including specialists and hospitals.
- Monitor medical transports to ensure seamless continuity of care.
- Manage hospitalized cases by reviewing daily clinical updates and communicating with the medical team.
- Assist in hospital discharge planning to meet patient care objectives.
- Coordinate higher-level care placements as necessary.
- Perform additional responsibilities as assigned by the regional office.
- Graduate from an accredited Nursing School.
- Current Registered Nurse licensure in the relevant state.
- Active CPR certification.
- Knowledgeable about state-specific nursing regulations.
- Completion of required clinical competencies as per role specifications.
