Community Support Specialist

We are seeking a highly motivated Community Support team member to provide comprehensive support services to clients with mental health issues and their families.
As a key member of our multidisciplinary team, you will be responsible for establishing and maintaining interpersonal relationships with clients and their families.
The ideal candidate will have a passion for delivering high-quality care in an institutional setting and be committed to making a positive impact on the lives of those we serve.
Key Responsibilities:- Participate in the intake process by collecting, organizing, and analyzing information about individuals or families.
- Provide direct casework services for social involvement, health needs, and community life to individuals or families.
- Prepare and collaborate on various reports and develop case presentations on assigned individuals or families.
- Perform regular visits to provide patient and family support and education.
- Coordinate appropriate programming, resources, and services for social development and in preparation for individuals or families.
- Serve as the point of contact in absence of the Manager, when applicable.
- Collect data and write brief reports to meet evaluation needs of the program.
- Complete IM+CANS Recerts.
- Make appropriate referrals to health and social service agencies.
Estimated Salary: $45,000 - $60,000 per year
