Responsibilities:
  • Collaborate with healthcare providers to individualize medication therapies and promote safe, efficacious, and cost-effective patient care.
  • Develop, implement, and evaluate quality improvement and clinical pathways/programs related to medication use to ensure optimal patient care.
  • Evaluate clinical programs/services provided and implement improvements using formal systematic methods of problem solving and streamlining to maximize value.
  • Contribute to formulary management and work with healthcare providers to ensure formulary compliance.
  • Participate in medical staff and service line meetings as requested, including Pharmacy & Therapeutics Committee, to meet patient care needs and Service Line objectives.
  • Assume a leadership role in Service Line initiatives as related to medications and the medication use process.
  • Develop, implement, and lead projected cost savings/cost avoidance related to medication utilization and provide routine reports on status.
  • Participate in Strategic and Operational Planning activities and the development of the Pharmacy Department's annual budget.
  • Educate, mentor, and evaluate pharmacists, pharmacy residents, and/or pharmacy students to develop and improve skills necessary for providing pharmaceutical care.
  • Develop and provide educational programs, learning modules, and knowledge assessment tools.
  • Educate other healthcare professionals regarding medications to promote optimal patient care via inservices, educational programs, face-to-face interactions, etc.
  • Review, interpret, and evaluate all medication orders for patients in all age groups.
  • Maintain patient medication profiles in an automated pharmacy information system.
  • Consult with and advise medical, nursing, and other healthcare providers regarding the prescribing, dispensing, or administration of medications.
  • Routinely monitor the patient's total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities.
  • Participate actively in cardiopulmonary emergencies.
  • Conduct assigned target drug monitoring programs and concurrent drug use evaluation activities as required.
  • Perform and document Pharmacist Intervention activities such as recommendations on drug product selection, dosage adjustments, and appropriate drug therapy.
  • Obtain medication histories on selected patients and communicate pertinent information to the appropriate healthcare provider.
  • Communicate with physicians, nurses, and other healthcare providers as necessary.
  • Prepare and/or verify the appropriate preparation of medications to be dispensed using the correct technique, as required.
Qualifications:
  • Registered Pharmacist with the Texas State Board of Pharmacy in good standing (RPH)
  • Aseptic Technique - Complete an ACPE approved 20 hour sterile products course within 90 days of hire
  • Post Graduate Year 1 (PGY1) Residency or 3 years of experience
