Two floating holiday shifts per year

Key Responsibilities:
- Assessment and Goal Development
- Delivery of Patient Care
- Education for Patients and Families
- Planning for Discharge
- Record Keeping
- Daily Operations Management
- Oversight of Assistive Personnel and Students
- Must hold a valid license in the state of Illinois
- Master's or Doctorate degree (or bachelor's degree prior to 2002) in Physical Therapy from an accredited institution.
- Must meet the annual stroke education requirement (2 hours per year) for PTs working with adult stroke patients
- Strong interpersonal and communication skills
- Required to work approximately one weekend per month or evenings based on assignment area.
- Ability to lift 50 to 100 pounds occasionally and/or 25 to 50 pounds frequently and/or 10 to 20 pounds constantly.
- This requirement supersedes any previous lifting requirement effective 08/01/2014. Must be able to lift up to 35 pounds without assistance. For patient lifts exceeding 35 pounds, or when the patient is unable to assist, patient handling equipment is to be utilized, with at least one other associate, when available. Unique patient lifting/movement situations will be evaluated on a case-by-case basis.
