We are seeking a compassionate and dynamic Registered Nurse to join our Hospice team at AdvisaCare Home Health Care. As a Hospice RN, you will play a vital role in providing emotional support and care to patients and their families in Northern Michigan and surrounding areas.
Key Responsibilities- Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
- Implement individualized plans of care and make revisions as needed.
- Ensure compliance with AdvisaCare Home Health Care policies and procedures, as well as Medicare regulations.
- Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns.
- Supervise paraprofessionals providing services to patients.
- Prepare clinical and progress notes, participate in interdisciplinary group conferences, and maintain high levels of customer satisfaction.
- Current Michigan license in good standing.
- Minimum 1 year of home care or hospice experience.
- Empathetic approach to patients and caregivers.
- Ability to adapt to patients' status and needs on a full-time, on-call basis.
