Revival Home Health & Hospice is seeking a dedicated Speech-Language Pathologist to provide essential one-on-one care in patients' homes while supporting their families.

As a Speech-Language Pathologist with Revival Home Health & Hospice, you will have the opportunity to grow your therapy skillset while enjoying the benefits of flexibility and autonomy.

Key Responsibilities:

  • Evaluate and treat patients with speech, language, cognitive, and swallowing disorders based on physician orders/referrals.
  • Review patient records for current diagnosis, past medical history, precautions, and contraindications.
  • Establish measurable goals and develop treatment plans to move patients toward their maximum level of speech, language, cognitive, and swallowing abilities.
  • Consult with medical providers regarding patient evaluation and treatment, progress, and discharge from treatment.
  • Maintain and submit evaluation, treatment, and billing documentation in the required format and timelines.

Requirements:

  • Master's Degree in Speech-Language Pathology.
  • Current Speech-Language Pathology licensure in the state and American Heart Association CPR certification.
  • One year of experience in speech pathology, with home health experience preferred.
