Responsibilities:- Assess, direct, delegate, coordinate, and evaluate patient care through collaborative efforts with healthcare team members.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide education and support to patients and their families to promote healthy behaviors and outcomes.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives to enhance patient care and satisfaction.
- Graduate of an accredited school of nursing with a current RN license to practice in the state of Illinois.
- Minimum of one year of experience in a psychiatric/mental health setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to prioritize tasks, manage multiple projects, and maintain a high level of productivity in a fast-paced environment.
