Responsibilities
We are seeking a skilled Clinical Social Worker Specialist to join our team. In this role, you will provide psychosocial support and other Social Work services to Hospice patients and their families in various settings, including inpatient units, home care, and nursing homes.
You will serve as a social work member of the Interdisciplinary Group (IDG) and act as a liaison between hospice staff and inpatient and nursing home staff. Your responsibilities will include developing social work plans of care, setting goals, and making recommendations based on patient/family needs.
Qualifications
To be successful in this role, you must have a Master's Degree from an accredited school of social work and at least one year of experience in Social Work in a healthcare setting. You should also possess knowledge of Word and Excel, New York State MSW licensure (LMSW or LCSW), and a valid NYS Driver's license.
