Key Responsibilities

  • Conduct home assessments to evaluate patients' living environments and identify potential safety hazards.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Document patient progress and maintain accurate records.
  • Participate in discharge planning and provide education to patients and families on post-acute care.
Requirements
  • Doctor of Physical Therapy (DPT) degree from an accredited program.
  • Current licensure to practice as a physical therapist in the state of California.
  • Minimum of 2 years of experience in home health or community-based physical therapy settings.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
