Community Mental Health Nurse Specialist

Join Suncoast Center as a Community Mental Health Nurse Specialist and contribute to the delivery of high-quality healthcare services in our community.
About the RoleWe are seeking an experienced and compassionate LPN to provide medical assessments, coordinate treatment plans, and administer medication to clients with severe mental illnesses. As a key member of our multidisciplinary team, you will work closely with psychiatrists, RNs, and other stakeholders to ensure seamless care coordination and excellent patient outcomes.
Your primary responsibilities will include:
- Conducting thorough medical assessments of clients' physical health and referring them to community physicians as needed;
- Coordinating psychiatric treatment with medical treatment under the guidance of team psychiatrists;
- Managing medication delivery records for each client, ensuring accuracy and up-to-date information;
- Ordering medications from pharmacies, arranging for delivery or pickup, and packaging medications according to prescription instructions;
- Collaborating with case coordinators and other treatment team members on client treatment plans, focusing on medication administration and education;
- Providing direct clinical services related to treatment plan goals, including medication administration, education, and intervention strategies;
- Completing all necessary documentation, including status letters, outcomes, FARS, treatment plan addendums, and reviews;
- Participating in daily staff meetings and treatment planning review sessions to discuss client cases and progress notes.
To be successful in this role, you will need:
- A valid Registered Licensed Practice Nurse (LPN) license in the State of Florida;
- Experience working with individuals diagnosed with severe and persistent mental illnesses, preferably in a community-based setting;
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including clients, families, and colleagues;
- Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines;
- Ability to maintain confidentiality and handle sensitive information with discretion.
The estimated salary range for this position is $65,000 - $80,000 per year, depending on experience and qualifications.
