As a key member of our healthcare team, you will conduct in-home health assessments, administer treatments, and develop personalized care plans to meet patients' needs and goals.
Key Responsibilities:
- Conduct Comprehensive Health Assessments: Evaluate patients' vital signs, medication adherence, and overall health status to identify areas for improvement and develop targeted care plans.
- Develop Individualized Care Plans: Collaborate with the healthcare team to create personalized care plans that address patients' unique needs and goals.
- Administer Medications and Treatments: Safely administer medications, treatments, wound care, and medical procedures following prescribed protocols and care plans.
- Monitor and Document Patient Condition: Regularly monitor patients' condition and document changes, promptly communicating updates to the healthcare team.
- Provide Patient Education: Educate patients and their families on disease management, medication adherence, and lifestyle adjustments to promote wellness and independence.
- Collaborate with Healthcare Professionals: Work collaboratively with physical therapists, occupational therapists, social workers, and other healthcare professionals to ensure a holistic approach to patient care.
Requirements:
- Current LVN License: Must be licensed as a Licensed Vocational Nurse in the state of Texas.
- Home Health Experience: Proven experience as an LVN in a home health setting, preferably with a focus on patient-centered care.
- Clinical Assessment and Patient Care Skills: Demonstrated proficiency in clinical assessment and patient care, with a strong focus on patient safety and well-being.
- Strong Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to Work Independently: Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Valid Driver's License: Must have a valid driver's license and reliable transportation.
