Job Summary

We are seeking a highly skilled Medical Social Worker to join our Renal Transplant team at St. David's North Austin Medical Center. As a Medical Social Worker, you will play a vital role in providing comprehensive social work services to patients and their families undergoing renal transplantation.

Key Responsibilities
  • Conduct psychosocial assessments and reassessments of transplant candidates and recipients to identify and address their social, emotional, and financial needs.
  • Provide education and support to patients and families on the psychosocial aspects of renal failure and kidney transplantation.
  • Collaborate with the renal transplant team to develop and implement individualized plans of care that address the unique needs of each patient.
  • Advocate for patients and families to access community resources and services that support their well-being.
  • Participate in multidisciplinary rounds and meetings to ensure seamless communication and coordination of care.
Requirements
  • Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited program.
  • Current licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of Texas.
  • Minimum two years of experience in medical social work, preferably in a renal transplant setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
