Located centrally in Baltimore, near the historic Mount Vernon area, UMMC Midtown Campus is a 180-bed facility focused on helping individuals manage chronic health conditions, including diabetes, hypertension, respiratory issues, and infectious diseases.

Position Overview

This role operates on a PRN basis with rotating shifts, including days and nights.

The Post-Acute Specialty Program (PASP) at UMMC Midtown Campus provides a nurturing environment for patients requiring care beyond the acute phase of their treatment.

PASP is composed of a dedicated team of healthcare professionals, including physicians, registered nurses, licensed practical nurses, patient care technicians, rehabilitation and respiratory therapists, dietitians, and other specialists who deliver tailored, continuous patient care.

The collaborative, multidisciplinary approach ensures that the goals of patients and their families are prioritized and met.

Key Responsibilities:

  • Ensure compliance with established policies and protocols while assisting colleagues in optimizing resource utilization. Participate in core measures, competencies, regulatory requirements, quality metrics, service excellence, and educational initiatives.
  • Serve as a unit leader, taking on charge duties, mentoring, and precepting new staff. Proactively identify and resolve issues that arise during shifts and in relation to ongoing nursing practices.
  • Conduct patient assessments and reassessments as necessary, collecting information from diverse sources to support the formulation of nursing diagnoses.
  • Develop individualized Care Plans based on comprehensive patient assessments, setting achievable and measurable goals, and meticulously documenting in patient records.
  • Coordinate and manage available resources and personnel to effectively implement and monitor the Care Plan.

Qualifications

  • Current Registered Nurse License in the State of Maryland.
  • At least 1 year of nursing experience.
  • ACLS provider certification is required within the first year of employment in designated departments.
