Job Description
The Hospital Care Coordinator role at BayCare involves providing linkage, monitoring, planning, and advocacy for individuals to achieve maximum functioning. This includes possessing a clear understanding of available resources to promote quality and cost-effective outcomes. The coordinator will effectively manage assigned caseloads, contingent on individual needs, strengths, abilities, barriers, and preferences. Discharge planning efforts will be coordinated through assessment, referral, and interdisciplinary planning, requiring the ability to adjust to changing priorities in a fast-paced environment.

Position Details
Location: St Anthony's Hospital - Florida
Status: Part-time, 16 hours per week
Schedule: Saturday & Sunday 8:00 AM - 4:30 PM
Weekend Requirement: Every
On Call: No

Required Skills and Qualifications
- Registered Nurse (RN) certification
- Associates of Nursing or Bachelor of Nursing degree
- 2 years of experience in Acute Care and as a Registered Nurse
- Preferred certifications include ACM (Case Management) and CCM (Case Manager)
