We are seeking a skilled and compassionate Case Manager Registered Nurse to join our team at Klamath Hospice and Palliative Care. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in need of hospice services.
Responsibilities- Assess and Develop Care Plans: Conduct thorough assessments of patients' physical, psychosocial, and environmental needs, and develop individualized care plans that address their unique requirements.
- Provide Skilled Nursing Care: Deliver high-quality nursing care to patients in their homes, focusing on pain management, symptom control, and overall comfort.
- Collaborate with Interdisciplinary Team: Work closely with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive care and support for patients and their families.
- Communicate Effectively: Maintain open and empathetic communication with patients, families, and healthcare team members to ensure seamless care coordination and address any concerns or questions.
- Document Patient Information: Accurately and thoroughly document patient information, including assessments, care plans, and progress notes, to ensure continuity of care and meet regulatory requirements.
- Education and Licensure: Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the state of Oregon, with ability to obtain licensure in California.
- Experience: At least one year of experience as a Registered Nurse in acute care, home health, community health, or hospice settings.
- Leadership and Communication Skills: Demonstrated leadership capabilities and excellent communication skills, with the ability to relate well to staff and other professionals.
- Computer and Technical Skills: Basic computer knowledge and proficiency in electronic medical records (EMRs) and other healthcare software.
