About the JobWomen In Need, Inc. is seeking a skilled Case Manager to join our Transitional Housing program. This is an excellent opportunity to work with a dedicated team of professionals who share your passion for supporting families in need.

In this role, you will work directly with clients to assess their needs, develop personalized service plans, and connect them with community resources to help them achieve stability and self-sufficiency.

Responsibilities:
  1. Conduct initial home visits to assess client needs and develop initial service plans.
  2. Provide ongoing case management services, including regular check-ins, goal setting, and progress monitoring.
  3. Collaborate with internal teams, such as housing coordinators and employment specialists, to ensure seamless support for clients.
Qualifications:
  • Bachelor's degree in social work or related field; Associate's degree with relevant experience considered.
  • 1+ year of experience in case management, preferably in transitional housing or a similar setting.
  • Strong communication, organizational, and interpersonal skills.
  • Familiarity with entitlement systems and procedures a plus.
