LiveWell Choice, a subsidiary of Longevity Health Plan, is seeking an Advanced Practice Provider - Nurse Practitioner or Physician Assistant to join our team in Port Chester, NY.
Job Summary:We are looking for a highly skilled and compassionate Advanced Practice Provider to provide primary care and services to our members in long-term care settings. The ideal candidate will have a strong background in geriatric care and be able to work autonomously while being part of a collaborative team.
Key Responsibilities:- Provide collaborative primary care and services to a panel of Longevity Health Services members, including treating in the most appropriate setting based on the member's goals of care.
- Execute and deliver Longevity Health Model of Care activities, treating in the most appropriate setting, and develop and implement advanced care planning.
- Facilitate and administer Health Plan clinical operations functions, including prior authorization, Part A and B services, Medication Reviews, HEDIS and STARs quality gap closure, and promoting membership growth.
- Serve as a trusted advisor to members, loved ones, and nursing home staff, and educate care teams on member needs and early identification of changes in condition.
- Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and care team members on members' care needs, goals of care, treatment plans/changes.
- State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required.
- DEA License required.
- Must have an active national provider number (NPI) and an active Medicare provider number.
- Must be eligible to participate in the Medicare program as a provider and registered.
