Essential Qualifications:
  • Completion of a Physical Therapy degree from an accredited A.P.T.A. institution or equivalent
  • Current licensure in Illinois or eligibility for licensure

Job Summary:

Under the supervision of the Rehab Services Manager or Rehabilitation Supervisor, and with accountability to the Director and Team, the Physical Therapist is responsible for conducting evaluations, creating treatment plans, setting goals, and developing individualized Physical Therapy programs aimed at enhancing patient mobility and promoting independence within their physical constraints.
