Job Summary:

The Social Worker shares the responsibility of identifying social and emotional needs of each client. The position assists with arranging services to meet the needs of residents, providing liaison between the client, family, facility staff, and the community.

Key Responsibilities:

  • Assess all patients admitted to the Behavioral Health department, then gather and analyze information from available sources including family, agencies, friends and others.
  • Formulate and communicate treatment information to other members of the interdisciplinary treatment team.
  • Participate in the development of the patient's treatment plan, coordinate or provide services including individual, group, family, marital and/or group psychotherapy and discharge planning.
  • Serves as a resource to staff and community regarding psychosocial issues.
  • Participates in Hospital and departmental planning committees and teams.

Requirements:

  • Master's Degree in Social Work from school approved by the Council on Social Work Education.
  • Current licensure as a Licensed Social Worker (LSW) or a Licensed Clinical Social worker (LCSW).
  • Current American Heart Association CPR Certification.
  • Proficient in Microsoft Office Software (Word and Excel).

Preferred Qualifications:

  • 2 years of hospital social work or relevant fieldwork placement in mental health field or home care.
