Job Summary

Deliver Exceptional Care

Lifespace Communities is seeking a skilled Licensed Practical Nurse to join our team in a community setting. As a key member of our healthcare team, you will provide high-quality care to our residents, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities

  • Participate in resident care planning and implementation, ensuring seamless delivery of services.
  • Document resident conditions and nursing needs accurately and in a timely manner, reporting pertinent observations and reactions.
  • Develop and direct nursing assistant assignments, adjusting based on census and level of care required.
  • Administer medications, monitor reactions, and document accordingly.
  • Initiate physician orders and facilitate communication with families regarding changes in medications or resident status.
  • Establish protocols to ensure care plans are continually reviewed and updated.
  • Coordinate admissions, discharges, and transfers to deliver quality customer service.

Requirements

  • Licensed Practical Nurse graduate from an accredited nursing program.
  • Certifications and licensure required by state regulations.
