Hospice RN Case Manager

At Interim HealthCare Home Health and Hospice, we are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families in need of hospice services.
About the RoleAs a Hospice RN Case Manager, you will be responsible for:
- Conducting ongoing assessments of patients' physical and psycho-social status
- Developing and implementing individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families
- Providing education and interventions to support effective pain and symptom management
- Ensuring timely and accurate documentation of patient services
- Participating in IDG meetings and making necessary revisions to the plan of care as needed
To be successful in this role, you will need:
- A valid, unrestricted RN license in the State of Michigan
- At least one year of relevant experience as an RN
- One year of management experience (preferred)
- One year of home health experience (preferred)
- Current CPR certification
- Reliable transportation with a valid state driver's license and current car insurance
