Job Summary

We are seeking a Youth Support and Development Specialist to join our team. As a Young Adult Case Manager, you will work with 6-8 clients at a time, providing individualized support and guidance to help them achieve stability and success in adulthood.

You will work closely with our Case Management Team Supervisor, Program Director, and Clinical Consultant to develop and implement service plans, document all case management activities, and collaborate with external stakeholders. You will also promote a strengths-based environment that fosters respect, empowerment, safety, and teamwork.

Key Responsibilities:

  • Provide one-on-one services to young adults, including home visits, transportation, and advocacy
  • Document all case management activities and maintain up-to-date case entry notes
  • Promote a strengths-based environment that fosters respect, empowerment, safety, and teamwork
  • Collaborate with external stakeholders, such as probation and child welfare agencies
  • Develop and implement service plans based on ongoing assessments of client needs
  • Write and submit timely reports for juvenile court and administrative reviews

Requirements:

  • Master's degree in social work, psychology, or a related field (Bachelor's degree with relevant experience may be considered)
  • Child Welfare Employee Licensure or ability to obtain within 6 months
  • Knowledge of childhood trauma and experience working with youth and families exposed to trauma
  • Valid Illinois Driver's license and access to a reliable vehicle

Estimated Salary: $68,400 per year.
