About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Haven Hospice. As a CNA, you will play a vital role in providing high-quality patient care and support to individuals and their families in their homes.

Responsibilities
  • Provide personal care services to patients, including bathing, dressing, and grooming
  • Assist patients with daily living activities, such as eating, toileting, and transferring
  • Monitor patients' vital signs and report any changes to the Case Manager
  • Assist with light housekeeping and laundry duties
  • Provide emotional support and companionship to patients and their families
Requirements
  • Current certification as a Certified Nursing Assistant (CNA)
  • Minimum of six months of experience in a hospice or home health care setting
  • Ability to read and follow written instructions and document care given
  • Strong communication and interpersonal skills
  • Ability to work with little direct supervision
