At Emblem Hospice, we are seeking a highly skilled and motivated RN to provide compassionate care to patients and their families in their homes and communities.
Job Summary:The registered nurse will plan, organize, and direct hospice care, with an emphasis on community health education and experience.
Key Responsibilities:- Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
- Provide professional nursing care utilizing all elements of the nursing process.
- Assess and evaluate patients' status, write and initiate a plan of care, and regularly re-evaluate their needs.
- Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
- Initiate appropriate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed.
- Counsel patients and families in meeting nursing and related needs and provide health care instructions.
- Assist patients with activities of daily living and facilitate their efforts toward self-sufficiency and comfort care.
- Act as a Case Manager and coordinate patient care for assigned caseload.
- Complete, maintain, and submit accurate clinical notes regarding patients' condition and care given.
- Communicate with physicians and other healthcare professionals to coordinate the care plan.
- Teach patients and families self-care techniques and provide a safe environment for the patient.
- Work in cooperation with the interdisciplinary group to meet the emotional needs of the patient and family.
- Participate in on-call duties as defined by the on-call policy.
- Supervise ancillary personnel and delegate responsibilities when required.
- Actively participate in quality assessment performance improvement teams and activities.
- Graduate of National League for Nursing accredited school of nursing.
- Minimum of two years of experience, preferably in public health, home care, or hospice nursing.
- Registered nurse with current licensure to practice professional nursing in the state.
- Demonstrates excellent observation, problem-solving, verbal and written communication skills.
- Shows ability to organize and prioritize workload independently.
- Maintains a current CPR certification.
- Must be a licensed driver with an insured automobile in good working order.
- Self-directed and able to work with minimal supervision.
