We are seeking a skilled Registered Nurse to join our team in Kaukauna, providing high-quality care to patients in their homes. As a Registered Nurse with Almost Family, you will have the opportunity to work in a dynamic and supportive environment, delivering exceptional patient care and building meaningful relationships with your patients and their families.

Key responsibilities of this role include:

  • Providing high-quality clinical services within the scope of practice and in accordance with standards of care.
  • Completing clinical nursing assessments as required by federal and state program requirements.
  • Developing and revising individualized plans of care to meet the needs of patients.
  • Ensuring plan of care frequency and duration meets the needs of patients and initiating revisions as needed with physician approval.

Requirements for this role include:

  • A minimum of one year experience as an RN.
  • Current RN licensure in the state of practice.
  • Current CPR certification.
