Optum Case Manager RN Job Description

Key Responsibilities:
  • Coordinate patient care processes to achieve desired quality outcomes and identify opportunities for resource optimization.
  • Directly communicate with staff, physicians, patients, and families to ensure seamless care transitions.
  • Facilitate patient and family education, promoting continuity of care and optimal patient outcomes.
  • Assure patient rights by offering choices and reviewing the patient plan of care with the multidisciplinary team.
  • Participate in multidisciplinary team care conferences for patients with complex problems.
Requirements:
  • Associate's Degree (or higher) or Master of Social Work (MSW) Degree.
  • Valid, unrestricted RN License in the state of Colorado.
  • Ability to work 5 shifts per pay period with one day being a weekend day.
Preferred Qualifications:
  • Bachelor of Science in Nursing (BSN).
  • Case Management Certification.
  • 3+ years of recent clinical or case management experience in the area of specialization.
